You are being invited to take part in a research study. Before providing your consent to take part, it is important that you understand what the study is about and what it will involve. Please take the time to read the following information.

This study is part of a research project for my MSc in Nutrition, Physical Activity and Public Health at the University of Bristol. I am looking at what motivates adults to do different types of exercise.

Taking part involves completing an online questionnaire which should take approximately 15 minutes. The questions will cover the kinds of exercise you do and your motivations.

It is up to you whether you decide to take part and you will be given the opportunity to provide your consent below. If after submitting your answers you change your mind, you can contact the researcher (details below) to request that your information is deleted without giving a reason. However, this might not be possible once the data have been anonymised (identifying information removed) and analysed.

Data is being collected using Online Surveys, which is fully compliant with UK Data Protection requirements. The information you give will be made anonymous, and stored in a secure online storage facility at the University of Bristol. All data will be deleted once I have graduated.

The anonymised results of this study will be used in my final research report and may be written up as a research paper. If you would like to receive a summary of the findings, please email
